成大資訊公告平臺KUAP 個人資料授權使用同意書

國立成功大學（以下簡稱"本校"）隱私權保護政策將幫助您了解，本校可能蒐集之個人資料、本校如何利用、保護個人資料以及本校告知個人資料的對象。 若您將個人資料提供予我們，將被視為您接受本政策。
依據個人資料保護法（以稱個資法）規定應向您告知下列事項。當您簽署本同意書時表示您已閱讀、瞭解並同意接受本同意書之所有內。若您未滿二十歲，應於您的法定代理人閱讀、瞭解並同意本同意書之所有內容後，本校始得蒐集、處理、利用個人資料、並遵守以下所有規範。請詳閱：

  1. 蒐集、處理及利用之目的： 本校為進行僅提供公共衛生事件及相關防疫措施及研究使用，向您蒐集個人資料，提供本校用於資料分析、學術研究及決策參考等用途。
  2. 蒐集、處理及利用之個人資料類別：
    身分證、姓名、出生日期、性別、email、電話及定位之相關數據。
  3. 個人資料利用之期間、地區：
    1. 期間：個人資料集之目的存續期間，依本校執行相關業務所必須之保存期間。
    2. 地區：不限。
  4. 依據個資法規定，當事人就其個人資料得行使下列權利，不得預先拋棄或以特約限制之：
    1. 查詢或請求閱覽。
    2. 請求製給複製本。
    3. 請求補充或更正。
    4. 請求停止蒐集、處理或利用。
    5. 請求刪除。

Personal Data Collection Agreement
National Cheng Kung University (hereinafter referred to as “NCKU”) Privacy Policy will help you understand what possible personal data will NCKU collect, how NCKU uses and protects personal data, and who NCKU discloses the information to.
Providing your personal data to us indicates that you have accepted this policy.

According to the Personal Data Protection Act (hereinafter referred to as the “PDPA”), you should be informed of the following. When you agree to sign this, it means that you have read, fully understood, and accepted the contents set forth in this agreement. NCKU is thus allowed to collect, process, use personal data, and comply with the following rules:

  1. Purpose of Personal Data Collection, Processing, and Use For public health, related pandemic prevention measures, and research use, NCKU collects your personal data to conduct data analysis, academic research, decision-making reference, etc.
  2. Types of Data Collected, Processed, and Used
    ID, name, date of birth, gender, email, phone number and location related data.
  3. Period and Area of the Use of Personal Data
    1. Period: The retention period of the personal information
      by NCKU shall be the time required for the above purposes for collection to be fulfilled.
    2. Area: Unlimited
  4. According to Article 3 of the PDPA, you may exercise the following rights with regard to your personal data and such rights shall not be waived or limited contractually in advance:
    1. the right to make an inquiry of and to review his/her personal data
    2. the right to request a copy of his/her personal data
    3. the right to supplement or correct his/her personal data
    4. the right to demand the cessation of the collection, processing or use of his/her personal data
    5. the right to erase his/her personal data
